Part 1:
You need to submit a functioning database, plus, ERD (make sure it is not hand-drawn, make sure you got enough entities- anything less than 7 is simply not feasible), Normalized data model- this is what you would have after resolving M:N's of your ERD. You would not need to normalize the existing files because you don't have any. Usually, normalization takes place when you convert, let us say, Excel spreadsheets to DB tables.
You got to have all your CREATE's- one per table. Make sure you declare FKs.
You should have at least one INSERT per table- no need do submit more than that- once you have the first one, they are all the same.
You should have the contents of your tables- this could be a screen shot, or, simply a table with records.
Part 2:
You need to have your forms- I suggested using MS Access, but you are welcome to use PHP, or anything else that you are comfortable with.
Part 3:
You need to have your SELECTs- one per report. You should be able to explain the purpose of each report. For example, you could say: "This report allows us to identify all the customers who spend over $5K on maintenance and repairs within last 6 mos."- and SQL code follows.

Please, realize that I would grade your submission based on the Guidelines. Make sure you have all the parts.
THIS IS REGARDING THE FORMAT:
Because you must submit the actual functioning SQL Server database, I suggest that you zip your SQL Server database and submit it that way. This will be your file #1. DO NOT ZIP ANYTHING ELSE.
Regarding the rest: do not submit a bunch of separate files- combine them into a single report. The report will contain all the parts: your ERD pasted, your queries pasted, your reports pasted, and so on. If you have your forms done using PHP/Java/etc., then paste the screenshots into the report also.
Please, submit the report separately- in a single MS Word document. This will be your file #2.
If you use MS Access to create your reports, then simply submit your database as well. This will be your file #3.
Consequently, you must have, at least, 2 files- File #1 is zipped- contains your SQL Server DB. File #2 is your Course Project report, in MS Word' format (DO NOT ZIP YOUR REPORT). File #3 is optional (only needed if you created your forms using MS Access)- this would be your MS Access DB containing your forms (DO NOT ZIP THIS FILE).
