Think about an associate or peer to whom you want to provide feedback. For instance, this could be a co-worker that is having problems taking on a new task where the associate must prepare reports by Friday of each week. This may remind you of a time when you had a similar responsibility added to your tasks and you had a struggle balancing your time to complete the task on time each week.
Write out a script of what you would like to initially state to them (please omit specific names or references that may shed light on the person/situation you are addressing). Be thorough, and you may need to anticipate and fill in what you think the other person will say. Try to not exceed 500 words.
