Assignment task: Nursing Diagnosis: Imbalanced Nutrition Less Than Body Requirement
Rita is an 87-year-old woman with moderate dementia who has lived in a skilled nursing facility for 6 months. She needs assistance with eating and is brought to the dining room for her meals along with the other residents who need assistance feeding. She usually sits in a geri-chair which puts her in a position not conducive to eating. The dining area is noisy with the television turned up very loud. She has lost 8 pounds since admission and her current BMI is 17.3.
She had a complete upper denture but it disappeared. Presumably, it was wrapped in tissue and was inadvertently thrown in the garbage.
The RN checked Rita's admission assessment in her chart to ascertain her favorite foods, but it was never filled in.
One of the nurses suggests a feeding tube, but Rita's nurse knows there are other steps to take before a feeding tube is inserted.
