Problem: Working with clients is a rewarding process because the circumstances and dynamics of each client's situation(s) are unique and individualized. Regardless of the types of clients being serviced by the Human Services Professional, a 'checks and balances' system of record-keeping is important for reasons ranging from proof of services provided to assessing the effectiveness of the work done with clients. Why is accurate documentation and record-keeping important (a) during the interview process and (b) when providing services to clients?
