At this point in the semester we all understand the value of a standard "medical" language. Another important component of medical terminology is documentation. Once you document a patient's symptoms, test results, exam results, etc., in a patients chart, the chart becomes a legal document. That is why it is so important that all health care providers document correctly and precisely.
Do some research on the internet on common documentation errors that have been found in healthcare? Choose one and indicate your thoughts on why this is a problem and possible solutions.
