Utilizing Electronic Health Records (EHRs), this initiative will help identify gaps in diabetes management and colorectal cancer screening. Additionally, we will collaborate with local Hispanic organizations to host health fairs and educational campaigns to increase awareness about diabetes management and cancer prevention (Rodriguez et al., 2019). Cultural competency training for healthcare providers will also be prioritized to address communication barriers and enhance trust in the healthcare system.
