Problem: A 68-year-old client with dementia has lived in a long-term care facility for 3 years. The client was admitted to the facility because the dementia had progressed to the point that the client was not safe ambulating within the home, was accusing family members of stealing, and was occasionally physically aggressive toward the spouse. The client's family is very involved and purchased a K0005 ultra lightweight wheelchair from a vendor because the client could propel it with ease. They also chose this model because the wheels come off easily for storage in a car when they take the client out for lunch each weekend. The OTR receives a referral to evaluate and modify the client's wheelchair system to prevent the client from falling out of the chair and to prescribe a cushion to help prevent pressure ulcers. The client's most recent fall resulted in a gash to the back of the head that required three stitches, and the client has a recently healed Stage 1 pressure ulcer on the sacrum. Which initial steps should the OTR take? Interview the client to determine the reason for falling out of the chair Interview nursing staff to determine why the client is falling
