Case:
59-year-old male patient with chronic hypertension and chronic asthma received level 4 emergency department evaluation and management services due to unrelenting headaches and blurred vision, and buzzing in his ears over several days. He called his primary care physician today and was told to go immediately to his local emergency department. Patient denies history of migraine headaches or recent fall. He takes Hyzaar for his hypertension and albuterol nebulizer inhaler for his asthma. The patient explained that he has noticed more wheezing lately due to the change in seasons, and so he has used his inhaler many times daily. The patient's blood pressure was normal at 120/75, lung sounds were clear, and the patient is not wheezing today. The headaches, blurred vision, and tinnitus over several days are attributed to the patient having overused his albuterol nebulizer inhaler in an attempt to clear his wheezing. He was provided with a corticosteroid inhaler to replace the albuterol nebulizer inhaler to treat his chronic asthma. The patient was provided with instructions for use of the corticosteroid inhaler, which included discontinuing use of the albuterol nebulizer inhaler. The patient was cautioned to use the corticosteroid inhaler only as prescribed. A follow-up appointment with his primary care provider was scheduled.
