Problem: The client is evaluated and admitted. The primary health care provider orders the following: 0.9% NS @150ml/hf 2 units packed red blood cells over 4 hours 4mg morphine IV now x1 Morphine PCA, 1mg Q10 minutes with a lockout of 6mg/hr Pantoprazole, 40mg IV QD IS 10 times per hour The orders are implemented. The nurse reassesses the client 4 hours later.
