Lesson Plan Format
1.	Student Outcomes:  In future lesson plans you will be writing outcomes to plan what you want to have your students accomplish.  Since you are being directed by the cooperating teacher you will write a reflective outcome.  Write outcomes in your own words according to what you accomplished with this lesson.
2.	Materials/Media/Technology:  What did you use in this lesson?  Note: videos and CD's are media, not technology.
3.	Assessment:  Indicate how each outcome in #1 was assessed.  Some lessons will have formal/summative assessments and others will have informal/formative assessments(this could be a simple as teacher observation or you could state how the information will be assess later by the teacher).
4.	Teaching/Learning Procedures:  This is your action plan for what you did with the students during the allotted lesson time.  Be specific regarding the following steps:
a.	Launch/Engage - How did you start the lesson (assessing prior knowledge, engaging/motivating students).
b.	Investigation - Questions you posed, possible student misconceptions, student groupings, etc.
c.	Differentiation - How did you specifically adjust the lesson content to create a more accessible and/or challenging task, even if students didn't require special accommodations.
d.	Closure - How students did share their findings and connections related to the outcomes in #1.
5.	Professional Reflection (DIG DEEP!):  Reflections are required after the lesson.  A professional reflection should include two components:
a.    Aspects that went well or went badly and comments about why and/or what to try differently if the lesson is repeated.
b.    What you learned from the assessment and how it will affect subsequent instruction. for those present.
