Utilize initial case management skills based on the client profile given. Read the case scenario carefully. Be creative and use critical thinking to ask yourself questions and fill in the blanks where you need to in order to help yourself make a more realistic case study (i.e. county=Harrow, Belle River, Kingsville, etc. or What other disciplines are involved at the agency in Windsor?)

The assignment should include the following:

Referral/Presenting Issues-The reasons for the referral to your agency.

Background Information -Individual circumstances, accommodation, income, social and economic factors, family and supports, cognition, other community agency involvement already? Are any other service providers already involved? Any other considerations?

Assessment -Your assessment of the presenting problems and prioritization of the key risks and needs that have to be addressed. Also include the possible strengths and assets which can be drawn on, as well as other services involved. Who did you talk to?

• Plan -A brief outline of your intended work. What you intended to do, what you managed to do, and in what order. Who did you talk to? Short and long-term goals with objectives of meeting those goals.

• Intervention - A short summary, review, and evaluation of your actions in terms of both outputs (what you did) and outcomes (the changes you may have been able to support or bring about).

Critical Analysis of Applied Social Work Values, Theories, and Approaches Used- What Social Work values, theories, and approaches did you use? What is important for you to address with the client?

Active & Passive Referrals Made- Did you need to make any referrals? Were any referrals suggested and left to the family to follow through if needed?

Next Steps- What would be the next step in working with this client? Is your job over after just the assessment/treatment plan? Explain.

Case Scenario- Mr. Smith

Mr. Smith is an 84-year-old man who was recently placed in a subsidized assisted living environment, directly after being discharged from the hospital after a stroke. He is having difficulty adjusting to his new living arrangements and having to share a room after living independently for all his life, and not wanting to move, even though he admits he can't live alone. He is referred to your agency in Windsor for some of his complex health needs, and possible query of dementia. The nurse on your multidisciplinary team sees a need for case management services upon her program intake and refers him to you. He has an assigned POA (his niece) with who he now has a poor relationship and no contact, and no other family or friends. He is a retired veteran, no longer driving, and placed in the county. He currently receives Old Age Security and a small pension from the Military. He reports being lonely, and depressed and states he cries all the time thinking about the past and the people in his life he loved. He is also a fall risk, and he enjoys walking to the local coffee shop two blocks away, his staff fears this is becoming unsafe. He also reports being suspicious of the staff at the assisted living home as he claims they "go through my stuff". The staff report that he has started getting aggressive both verbally and physically as he has attempted to strike out at the staff and his roommate, and has recently been given 60 days to evict the assisted living home for biting a staff during an altercation. He is open to your services as he admits he needs help, as he will be homeless soon.
