The initial speech-language evaluation revealed slowed processing speed and memory impairments affecting performance of lengthy and complex tasks. Neil answered concrete and abstract yes/no questions and followed two- and three-step commands, but he had difficulty with four-step commands. When read a paragraph, Neil immediately answered simple yes/no questions about the story with 80% accuracy. However, when asked to retell the story immediately after hearing it, Neil could not generate either the main idea or details. Further testing revealed that he had stored a portion of the concrete story information in memory but had difficulty accessing that stored information to generate the story retell. His expressive language abilities were largely intact, and he spoke in complete, grammatically accurate sentences. Neil performed sentence completion, object naming, and picture description tasks adequately, and his abstract language comprehension was functional. Given extended time, he generated multiple solutions and solved common as well as novel problems. Neil's speech was slightly imprecise, slurred, and hypernasal. He used several strategies, such as slowing down and taking frequent breaths, to increase speech intelligibility. At times, Neil expressed frustration about the way his voice sounded.
