Problem: The nurse is caring for an 84-year-old client who has had enteral nutrition through a PEG tube for the past month. The enteral nutrition has been discontinued and he has started to eat regular food. He is prescribed 50 mL of water every 4 hours to keep up his fluids and keep the tube patent. Three days later he is exhibiting muscle weakness, lethargy, and confusion.  BMP shows Na+ 128, K+ 3.5, Cl 88, BUN 20, Cr 1.2.
