Barbara Witten, a 67-year-old female client (DOB: 4/20/1957), has been taking methotrexate for RA for the past two years, with doses steadily increasing to attempt to achieve a therapeutic response. She has been taking the maximum weekly dose of methotrexate for six weeks and is still experiencing moderate disease activity. The physical exam is normal outside of joint pain and swelling. In addition to non-pharmacologic interventions, NSAIDs, and glucocorticoids, the NP considers other pharmacotherapy to prescribe for RA.
