A 68-year-old man who is obese and has a history of previous stroke with mild residual, left-sided hemiparesis was left at home for "few hours" by his relatives. A neighbor called for help when no one opened the door of the house. The environmental temperature was noted to be hot. When help arrives, the man was confused and his body temperature was 40 °C. He was tachypneic, tachycardic, and hypotensive. He had facial flushing and crackles in the lung examination. A list of his medications included aspirin, lisinopril, atenolol, rosuvastatin, and oxybutynin. Within minutes the patient develops tonic-clonic seizure, which lasts for about 45 seconds.
