During my clinical experience, I worked with a patient who had been experiencing increased suicidal ideation over the past few weeks. He had not been staying at home, opting instead to stay with friends. Several days prior to his hospital admission, he had taken 120 mg of Escitalopram along with a handful of Tylenol. The patient reported consuming alcohol 1-2 times a month and using marijuana 1-2 times a week. He also experienced hallucinations, including hearing voices and seeing ghosts.
The patient struggled with body image issues and felt guilt around eating, often restricting his intake with the goal of losing weight. He typically only ate lunch or dinner and sometimes induced vomiting. My patient is a 15yrs who is transitioning to a preferred gender. being a 15-year-old, he is at the developmental stage of adolescence and this is a stage of identity verus role confusion. Throughout the two days I worked with him, I was intentional about using the correct pronouns, which was a new practice for me, given my different background. I had been taught to adhere to traditional gender pronouns, so this was a learning experience. My own beliefs, shaped by my heterosexual relationship and religion, influenced my understanding of gender pronouns. However, I made a conscious effort to provide holistic care by putting my biases and recognizing the importance of cultural safety, humility, and competency. Using the correct pronouns not only advocated for the patient's rights but also helped build a therapeutic relationship, fostering inclusivity and creating a safe environment. I aimed to encourage and validate his experiences throughout our interactions.
