What do you feel are most important health issues to discuss

Question: As you start your shift, you meet your patient Juan. He immigrated from the Dominican Republic 3 years ago. He was admitted to the hospital with blood sugars above 400 mg/dl (Normal Fasting blood sugar is below 100). His MD diagnosed him with Type 2 Diabetes and referred him to the Emergency Room. In his chart, you notice upon admission, his daughter said "he has been urinating a lot and has been feeling dizzy." HIs daughter stated "my father has gained 30 pounds because he has been eating low cost frozen meals because that is all he could afford." As you introduce yourself to Juan, you realize he is unable to answer your detailed questions because of his limited English fluency. How do you communicate and help a patient that does to speak English fluently? Looking at his medical chart, what do you feel are the most important health issues to discuss?
