Problem
Construction worker presents to the ED with complaints of chest pain, dyspnea, and nausea. Patient has a past medical history of Prediabetic, hypertension, GERD, COPD, social smoker, and drinker and is noncompliant with his medications.
What differential diagnoses would apply to the patient, as well as the rationale for why the differential diagnoses would or would not pertain to the patient? What labs would you be concerned with? What imaging would you order?
