Akira, a new patient to the obstetric practice where you work, is waiting in the examination room for the certified nurse-midwife (CNM). Akira is 28, a G2P1, and at 7 weeks 3 days gestation dating from her last menstrual period (LMP). Akira met her active duty husband while he was stationed in Japan 5 years ago, and her mother and brother came to the United States along with them after his deployment a few years ago. Akira and her husband have a 3-year-old son together who is still unaware that his mom is pregnant. As you wait for the CNM, you reflect on your conversation with Akira. She's told you that she has a great relationship with her mother and can't wait to share her joy at the delivery with her as she did with her son when she delivered in Japan. However, unlike Akira herself, who speaks English very well, her mother doesn't speak any English and is very traditional. She is expecting the same experiences and traditions to be followed in this delivery as they were with Akira's first, and Akira is having a hard time explaining the differences. Akira is worried that her mother will be upset at some of the Western traditions and asked you if her mother could accompany Akira on a few of her appointments to learn more about what she may see at the delivery. What is your best response to Akira? Of course, you can bring whoever you want. But we won't have time to explain the care to your mom; your appointments are too short. You can translate to her whatever is said to you as the CNM goes through your appointment. O Don't you think it's more important for your husband to be here? Of course, you can bring whoever you want to your appointments. Can you tell me more about the traditions we should focus on addressing that are of the greatest concern for your mother?
