What additional diagnostic tests would be helpful

Case exemplar: Patient With Abdominal Pain and Fever PM is a 14-year-old female complaining of abdominal pain. She was in her usual state of health until about a week ago when she began noticing a dull cramping pain in the right lower quadrant of her abdomen. She has had a low-grade fever on and off and has noticed an increase in the frequency of her stools. This morning there was blood in the stool, so her parents called the office for an appointment. She is afraid of having appendicitis and is afraid of surgery. She denies nausea, vomiting, or skin rashes. Menarche was about 6 months ago, and her menstrual periods have been normal. She denies sexual activity, and she denies recent travel. What are the top three differential diagnoses for this patient? What additional diagnostic tests would be helpful? How should PM be treated?
