Follow the steps below and answer each of the questions. For each question, provide the proper descriptions and explanations in at least 1-2 sentences.

1. Why is a PHR a valuable tool?
2. Outline the steps that you would need to take in order to create your own personal health record (or one for a family member). Consider the variety of health care providers that you consult (or have consulted).
a. Do you have contact information such as addresses, phone numbers?
b. Would you need to collect record copies? From whom? Of what?
c. How will you obtain these?
3. Describe the type of information you would want to put in your (or loved ones) PHR.
4. Decide the medium you would likely use (paper, web-based, software based, etc.).
5. Identify who you would notify about your PHR.
a. Who will you give access to?
b. How will they locate it?
6. Describe how the emerging PHR concept might impact the HIM professional. Could there be new jobs created relating to PHR?
7. Now go to the Tools and Resources tab and select the Chose a PHR option. Scroll to the bottom of the page and click on Web-based.There may be a slight delay but you will see below a long list of PHR options.Choose one and explore it.
