Problem:
A 50-year-old black male patient, was seen in urgent care with complaints of fever, chills, chest pain, cough, common cold like symptoms, nausea, vomiting, headaches, abdominal pain, and diarrhea. He stated the onset of these symptoms have persisted for 1 month. Specimen collection was collected via a sterile dacron swab to trap upper respiratory microorganisms. Direct visual examination for upper respiratory infections consisted of identifying yeastlike cells that would indicate thrush. This is done via gram crystal violet staining and yielded negative results. Sputum was collected via a deep cough in a sterile container for lower respiratory tract infection testing. Direct visual examination for lower respiratory infections consisted of direct wet preparation to look for parasites and special procedures for Pneumocystis. Additionally, a fixed and stained specimen was completed to identify bacteria and yeast that was recognized via Gram staining. Legionella spp. was found to be the causative agent in the sputum.
