Tenderness between the first and second toe


Problem: A woman arrives at the clinic because she has had a sore foot for weeks. She said that she went hiking in sandals, during which she stubbed her toe. She noticed no other injuries at the time, but she said that it was hard to see the bottom joint of the toes and that she did not really look closely anyway. Since then, she has had continued pain and tenderness between the first and second toe. The space between the toes looks swollen and erythematous. Some purulent discharge is evident in the space between toes. Propose your next step. Order a soft-tissue X-ray to look for any foreign object as a site of infection. Purulent exudate indicates tinea pedis. Advise her to use some antifungal cream between the toes nightly and to keep the foot dry. She should wear only white cotton socks at all times and avoid shoes with tight toe-boxes or overly high heels. Advise topical antibiotic ointment to be applied twice a day, and tell her to soak the foot in Epsom salts twice a day and keep it elevated in between times.
