As mentioned in the week seven lecture, an annotated bibliography is merely an enhanced works cited page. In the example accessed through the link below, you will notice that the bib is broken into three parts: (i) a summary of the text, (ii) an assessment of the text, and (iii) a reflection on the text. So, you summarize the text you have read; what are the main points, etc., then you assess it for its credibility--is it credible and HOW DO YOU KNOW? Then you will reflect on ways you will use it in the paper, i.e. will it support your thesis? Will it support one of your other sources? Will it add information about the topic?
