Strategies for rising health literacy in diabetic patients


Problem: Compare and One of the main quality improvement strategies for increasing health literacy in diabetic patients involves coordinating multidisciplinary care and increasing patient and family involvement (Ginnard et al., 2020). For these patients, multidisciplinary team members can include the patient, supportive family members, physicians and medical staff, pharmacists, educators, and dietitians. As patients are often overwhelmed at doctors visits due to the volume of information shared, it is vital to have a support system for them to rely on. As the majority of time is spent within the patient's home, family members play a key role in encouraging compliance with diet and medications. Programs such as Diabetes and the Family, that focus on educating both patients and family members lead to better health outcomes, improved health management, and reduced health complications (Centers for Disease Control and Prevention, 2018). Education and implementation of the use of continuous glucose monitors (CGMs) are another form of quality improvement for diabetes. As diabetics often feel overwhelmed with being asked to check their blood glucose multiple times throughout the day, the integration of CGMs into these patients' lives can improve their self-management of their disease and reduce risk of complications and hospitalization as a result
