Rewrite polished version of this report: During my observation, I noted that the biological mother exhibited continuous signs of possible substance impairment. Specifically, I observed her "nodding off" while seated in the waiting room, attempting to complete two new patient packets. This observation persisted for approximately 20-30 minutes. In an effort to assist, I approached the mother and inquired if she needed help completing the paperwork, as she had been working on it for about an hour. The mother did not respond to my offer of assistance. Instead, she asked her adult brother, "AJ," for his glasses, stating that she was having difficulty seeing. Despite this, she continued to struggle with the paperwork and exhibited signs of "nodding off." and had great difficulty concentrating on completing the paperwork.
