Problem: Select the findings in this clinical situation that put the client at risk for respiratory arrest. Upon review of the client's electronic health record (EHR), the primary care provider reviewed that day's medications to identify possible causes of this sudden change in respiratory status. They discovered the client had received oxycodone three times in less than 8 hours (one dose prior to the dressing change and two doses after). The charge nurse checked the automated dispensing system (ADS) and found a higher concentration of oxycodone solution stored. Consequently, the dose administered to the client was almost five times what was ordered.
