Example of how to end incident report about Subject: Incident Report Regarding Pulmonary Resident's Action During Rapid Response I am writing to formally report an incident that occured during our Rapid Response 9/28/2024 9:45am. The incident pertains to the administration of oxygen therapy using non-rebreather mask. During the Rapid Response, the patient required immediate oxygen support. Patient is desatting to 60's. Floor nurse placed the patient in non-rebreather. According to our hospital policy and clinical guidelines, a non-rebreather mask should be set at a flow rate of 12-15 L/min to ensure adequate oxygen delivery. However, Dr. Gandhi Pooja insisted on setting the flow rate at 6L/min, which is significantly below the recommended range. Despite the Charge RN, advising Dr. Pooja to adhere to the standard protocol and set the flow rate at 12-15L/min, Dr. Pooja persisted with her decision, stating, 'I want it that way" The deviation from established guidelines could have compromised patient safety and the effectiveness of the oxygen therapy. The incident raises concerns about adherence to clinical guidelines and the potential impact on patient care. I recommend a review of this case to ensure that all staff members, including residents, are fully aware of and comply with
