Introduction Jim Cooper a 68-year-old man was admitted to hospital with 1-week history of cough and shortness of breadth. He is a retired truck driver, a widower, with two daughters and a son. He has a history of smoking for about 45 years but quitted 7 years before. His history reveals that he has COPD diagnosed 7 years ago, Reflux and Hypercholesterolaemia.
His check up suggests that he has a blood pressure is on the higher side (136-140/88-90 mmHg) than normal (120/80 mmHg). The respiratory and the heart rate are also high compared to normal. The heart rate is in the range of 94-104 beats / min and the respiratory is 28-36 beats/min. While the normal is 60-100 beats/min for heart rate and 12-20 beats/min for respiratory rate. The Saturation oxygen level is 88% only (normal is 97-99%). The partial pressure of oxygen dissolved in arterial blood is 77 mmHg, slightly lower than normal 80-100 mmHg. But the amount of Carbon-di-Oxide dissolved in arterial blood is 55 mmHg, which is higher than normal of 35-45 mmHg. His WBC count is normal 3.8 x 109 /L also the temperature is close to normal 37.50

C. Level of Sodium and Potassium also looks normal with 133
mol/L for Sodium and 3.7 mol/L for Potassium. Although the Creatinine level looks high 75 Mol/L and the normal range being 53-115 Mol/L (Global rph 2013).
Based on all of these diagnoses three diseases of Mr. Jim three health issues of him: COPD associated Pneumonia, Hypercholesterolaemia and Reflux.
