The increase in coronary flow has been termed flow reserve and its magnitude is important in providing adequate perfusion to the working myocardium when hypertrophy due to hypertension has occurred, the capacity to increase flow to adequately meet the demands of this hypertrophied muscle is often markedly reduced. Thus patient with hypertension may have ST depression and angina during exercise testing, even when they have normal epicardial coronary arteries.
