Laslo 68 years old male residing in a rural area is a retired farmer. Laslo has a history of AKI 12 years ago due to dehydration and heatstroke. Despite of recovery from AKI, his kidney never fully regained their function, leads to CKD. His glomerular declined to 10 mL/min/1.73 m. Late afternoon he present to ED with mummble. BP is 96/60 mmHG with MAP 73 mmHG, UO is <400 ml per day abd has not voided during 24 hrs. creatinine is 900mmol/L. Blood Urea Nitrogen is 16.06mmol/L. DiLysate is 300. Doesnot smoke, alcohol consumption is less than 1 standard drink per wel, weight increases, BMi 23.5.Medications perindophrul, amplodipine, rosuvastine, furesomide, epeotein alfa, calcutriil, dietary modification of low potassium abdominal low sodium (<6 g/d) diet, proteint intake of 1.02-1.2 g/Kg/day.
