Problem:

Marlee is a 57-year-old on the medicine unit with uncontrolled diabetes. She was brought to ER three days ago with blurred vision, increased urination, and mild confusion. Her blood sugar was elevated at 28.3 mmol/L. She was admitted for intense diabetes management. Marlee has been on sliding scale insulin since admission, and today the glucometer has readings between 10.2 - 12.3 mmol/L. She has had diabetes since childhood, Type 1, but recently due to a mental health challenge has not been taking her insulin as directed. Her daughter noted the changes in her mom and called 911. She has no other medical history except allergies to her cats fur and grass. Today, Marlee says she feels nausea and does not feel like eating. She also complains of a headache. Her glucometer currently is 10.9 mmol/L. The nurse reviews the patient's chart and notes there is no order for an anti-emetic or analgesic. The nurse notifies the physician and requests an order. Provide SBAR report to the physician.
