Problem:

A mother brings in her 15-month-old boy who has a sore, painful right thigh. The child received immunizations in that site 48 hours ago and subsequently developed a low-grade fever, was fussy and irritable, but has fed well and slept without difficulty. He has been pinching at his leg at the site of injection, and there is an area of erythema that is 4 cm in diameter at this site. There is some localized increased warmth in the area to touch, but there is no induration and no centralizing wound. The mother is furious and certain that her baby was given the immunization incorrectly, somehow causing this child to have "an infected leg." She tells you that she intends to "sue the pants off" the nurse who administered the immunization. Her baby has had immunizations before but never was as upset as this. Even his leg never became red like this. She is sure that the nurse must have done something horribly wrong. She wants her baby to be checked out thoroughly with complete laboratory tests and X-rays and maybe a CT scan. Propose a response to the mother's concerns.
