Writing an Intervention Plan:
Goal and purpose of the assignment:
The goal for this assignment is for each student to produce a written intervention plan. The purpose of this assignment is for each student to gain familiarity with the processes associated with developing a plan for intervention with clients and writing a narrative based upon this plan. This notebook assignment builds upon the work completed in Notebook Assignment #3. Keep in mind that Notebook Assignments #3 and #4 will serve as the foundation for the final paper in this course.
Instructions:
- Present the Formulation of the Problems/Needs section from the Notebook #3 Assignment. From the feedback that you received from your instructor regarding the
Notebook assignment, be sure to make necessary corrections and identify four to five significant issues/areas to be used as goals for the intervention plan.
- Develop 2-4 well constructed paragraphs that present the plan you and your client have developed
Intervention Plan Components:
- Describe the goals, objectives, and time frames
- Problem
- Goals = results the client hopes to realize
- Objectives = steps toward meeting the goals. Objectives should be specific and measurable. In addition, objectives should be achievable by the client and realistic.
- Intervention = the strategies used to work on the objectives. Some of the strategies we have touched upon in this course have included parenting education, the ABC technique, and communication/social skills development strategies.
- Time frames = provide a reasonable estimation of when the objective will be achieved
- Identify and include an environmental challenge that affects the client and suggest a large-system strategy that you might employ to address that challenge. For instance, if poverty plays a role in the client's challenges, what might you, as a professional social worker, set as a goal for addressing the issue of poverty in the client's community? What objectives would you set? What interventions or strategies would you use? What sort of time-frame would you set for yourself?

- Should be 2-3 typewritten, double-spaced pages (not including the title or reference pages).
