Problem: Karen is a 48-year-old woman here today for c/o painful urination x1 day with mild low back pain. She denies blood in the urine or vaginal discharge, but does have pain with intercourse. Has noticed some blood on the tissue after wiping. GC/Chlamydia negative last week. Uses condoms for birth control. LMP 2 weeks ago. Vital signs are 108/82, pulse 128, RR 30, and temperature of 99.6 F. UA shows trace protein, urobilinogen=3.5, urine nitrate is positive, WBC 1 , pH 6.0, specific gravity 1.025
