Joshua has recently experienced an increase in behavioral problems since his middle sibling was placed in the home with him. His grandmother reports that Joshua becomes highly reactive when he perceives that his sibling is receiving more attention, leading to episodes of jealousy, anger, and mood shifts. These outbursts often carry over to school, where he displays emotional instability, difficulty concentrating, and struggles to focus on tasks, especially in his least favorite class, remedial reading. He is reported to be at least two years behind in reading. Despite these challenges, Joshua is talkative and social with his peers when in a happy mood. He has a strong interest in Art, which is his favorite subject. Joshua's grandmother has become increasingly overwhelmed with his emotional outbursts, noting that he is often restless and irritable. She reports that he has difficulty sleeping, poor concentration, and episodes of edginess. Joshua's mood can rapidly shift from being constricted to volatile, with frequent angry outbursts that interfere with his relationships at home and school. Joshua was recently diagnosed with Oppositional Defiant Disorder (ODD) and Attention Deficit Hyperactivity Disorder (ADHD).
