Your patient is a 70-year-old homeless Spanish speaking man with HTN, DM II and Kidney Failure and has absolutely no form of resources and does not know where to get help. You will need to advice your patient on where to find resources within the community. 1. IV.C7 Identify resources and adaptations that are required based on individual's needs.
