Problem: Martin, a 43-year-old man, suffers an embolic stroke. He had been treated for a damaged mitral valve, which probably served as a site for thrombus formation (blood clot formation) in his heart. Small fragments from this thrombus broke loose and traveled to his brain, blocking off some small vessels and at least one medium sized branch from the right middle cerebral artery. He received thrombolytic therapy upon arrival at the emergency department and was admitted to the ICU. His blood pressure was 190/105. A few hours later he began to experience a severe headache on the right side of his head and paralysis of the left side of his body, followed by a seizure.
