Probable diagnosis of community acquired pneumonia


Problem: A 42-year-old woman presents to the hospital with a probable diagnosis of community acquired pneumonia. Her chest x-ray shows a large pleural effusion. The nurse practitioner orders/performs a thoracentesis with fluid results as follows: Color- viscous and cloudy, pH- 7.11, protein- 5.8g/dl , LDH-285IU/L, glucose- 56 mg/dl, WBC-3800/mm3, RBC-24,000/mm3 Gram stain- many PMN's, gram + organisms seen.
