Primary care physician for diarrhea and vomiting


Problem:

Harper Anderson is a 10-month-old female who presented to her primary care physician for diarrhea and vomiting the past two days. She had two large, loose stools on the first day. Her mother reports that she is less active, not interested in playing, and is sleepier today. She is unable to keep any feedings down today. She had four loose, watery stools and emesis x3 this morning. She has not had a wet diaper since yesterday. She is 28.5 inches (72 cm) in length and weighs 8.5 kg. She weighed 9 kg at her last office visit two weeks ago. Harper is being admitted to the pediatric unit for further care. Past Medical History: Harper was a healthy full-term infant that weighted 3 kg at birth. There were no complications with the pregnancy. No past health problems. Mom is no longer breastfeeding and Harper is on formula. NKA. Harper has had no immunizations since birth. Social History: Harper's mother is 19-years-old and single. Harper is her first child.
