Patient admitted to a childrens hospital burn unit


Problem: A 10-year-old girl, Ellie (preferred pronouns: she, her), was admitted to a children's hospital burn unit for a 50% total body surface area burn on her face, upper chest, and back. She was being treated with oral acetaminophen every 4-6 hours and ibuprofen, a nonsteroidal anti-inflammatory drug (NSAID), as needed for pain and discomfort. The child had dressing changes and burn inspection every other day. Ellie was also given oxycodone prior to dressing changes, which has been therapeutic in providing pain relief and comfort, allowing her to rest on those days. After five days in the hospital, the mother noticed Ellie's breathing "was different" after the last dressing change. The mother pushed the call light and the nurse entered. The mother stated, "I put my hand on her chest and it doesn't move much. I've never seen her this sleepy."
