Using the following information for five different hypothetical cases, perform the Nutrition Care Process as if you were the RDN at a WIC site. ADIME note using our ADIME template (including a PES Statement, Intervention, Monitoring & Evaluation). Focus mostly on the Diagnosis, Intervention, Monitoring and Evaluation sections; you may include n/a for any Assessment data sections you are not able to obtain. Include within the ADIME note any possible referrals to other professionals and/or services if the needs of the client are beyond individual scope of practice. Include justifications for any referrals made.
