You are employed as an outreach Disability Case Worker working with individuals that have recently transitioned into independent living in the Community. In this simulated scenario this is the first time that you have met with the client. The information you have received was contained in a referral from the manager of his previous group home. The referral information is summarised as follows: Individual's name is Don, he is a 43 year old male who has recently moved out of a Supported Residential Facility located in Noarlunga (group home facility) into Independent Living in Christies Downs 3 weeks ago. Don is feeling very lonely and quite sad since moving into his own flat. He has been smoking twice as much as usual, he hasn't left the house in more than 2 weeks because he does not know where the bus stop is. Don has an intellectual disability. Don has a sister and a brother that live at Noarlunga - he has not seen them since he moved, as he does not know where the stop is or which bus to catch. He expressed a desire to find some employment in the weeks before leaving his group home.
