Assignment Task:

Your patient is Denise, a 17-year-old woman, has been bought in by her parents to the emergency department by car after syncopal episodes on standing at home and appears drowsy. She has a history of anorexia and of self harm in the form of cutting her arms and legs. On one occasion Denise has attempted suicide by cutting her wrists.

On presentation reported symptoms of amenhorrea, decreased urine output, constipation and generalised weakness. She is orientated throughout her emergency admission, with a GCS of 15. She has no fresh wounds, but there are multiple scars to both her lower arms and upper thighs.

She is dressed in a tracksuit and hoodie on arrival, but the emergency staff have changed her into a hospital gown. Her hair is lank and greasy, and appears unwashed. Denise is not visible dirty, and does not smell unpleasantly. She appears very slim for her age, and you can see all of her ribs and spine at the back of her hospital gown. Her BMI is 15.

You are working on the paediatric ward, and Denise is bought to you from emergency with an IV cannula in situ. She is receiving Normal Saline (Sodium Chloride 0.9%) with potassium added IV. Denise's father is with her, her mother has left the hospital to look after Denise's siblings who are younger than her.

After interventions in emergency, Denise is physiologically stable. The nurse handing over to you apologises and says "it's been totally crazy in the ED tonight, I'm sorry but we have done no assessments other than an A2K. Can you look after that from here?"

Denise is initially quiet and compliant with all care, however, she refuses to look at her father or respond when he speaks to her. Denise's mother telephones the ward and asks to speak to you. When you answer, her mother states that Denise's behaviour is very disruptive for her other children, and after this admission she just can't take it any more. She demands that you have Denise admitted to the secure mental health unit in the next town as an involuntary patient, and refuses to let her come home.

After speaking about the circumstances leading up to her admission, and after her father informs her she is no longer welcome at home, Denise became increasingly tearful and distressed, and started demanding to see a doctor. After being told that the doctor was on their way and would arrive in about 10 minutes, Denise became very distressed and dislodged her IVC.

She ran to the door of the ward and then sat on the floor, with her knees drawn up to her chest. She started sobbing and saying "I'm a terrible person, I don't deserve to be here, I just want to go home" repetitively.
