Name at least four ethical code violations

Assignment task:

Dr. I M. Sleaze has been seeing a couple in psychotherapy for marital problems.  The husband is very passive and this irritates the wife, who is a "go getter".  The husband is employed by a company that is not encouraging his advancement.  The wife is "stalled" in the middle of graduate school, not being able to finish her dissertation in Counseling. Dr. Sleaze administers an MMPI to both of them and tells the wife that she scores high on the "Hostility to Men" scale, and that the husband scores high on the "Passivity" scale (note that neither of these is a real MMPI scale). Dr. Sleaze then goes on to say that, based on these results, the husband should quit his job, and open his own company to overcome his passivity.  He tells the wife that she has to finish her dissertation.

Dr. Sleaze offers to help in the following ways: He will become CEO of the husband's new company, and will continue to see the husband in therapy, but since he is now CEO, he will call it "Industrial Consulting" instead of therapy, though the meetings will be the same as the therapy sessions.  Of course, since Insurance companies do not reimburse IO Consulting, the husband will have to pay out of pocket.

The husband runs into some cash flow problems in his new business and Dr. Sleaze offers to loan the man some money to help him out financially; He charges 28% interest. When the man falls behind in his payments for the loan and his therapy sessions, Dr. Sleaze terminates therapy (without making a referral for after-care) and sues him.
