Problem:

Your nurse comes to you and mentions that there are no more antibiotics in the back-up for Mrs. F. The pharmacy will not be able to replenish the medication until later in the evening, which will result in her missing two doses of the antibiotic. However, another resident, Mr. T, is on the same type and dose of antibiotic. What should be the nurse's next steps? Have the nurse use Mr. T's medication and have the two doses replaced into Mr. T's medications when the medications are delivered. Then notify your medical director and QAA committee of this occurrence at the next meeting. Have the nurse access your contingency plan (back-up pharmacy). If that is unable to deliver, have the nurse notify the MD and request a substitution for Mrs. F's antibiotic. Notify the responsible party of the need to change antibiotics. Then perform a root-cause analysis as to why/how this occurred, to prevent reoccurrence. Have the nurse notify the medical director and request a replacement antibiotic, then notify the MD and family of the change of antibiotics. Then report this occurrence to your QAA committee at the next meeting. Have the nurse skip the dose of the antibiotics
