Problem:
A 62-year-old male presents worsening hand tremor and recent falls. He noticed it initially approximately 2 years ago in his right hand, but has since progressed to both hands and occasionally his jaw. In the past several months he developed stiffness and movement difficulties in his arm and leg and has been having more difficulties with his balance and coordination. He has fallen twice in the past three months. His symptoms are aggravated by fatigue and seem to worsen as the day progresses. He reports some relief with rest. He denies pain although reports this is affecting his ADLs. He has no prior history of neurological disorders.
