In this assignment, you will demonstrate how to conduct a hazard risk assessment by applying unit concepts to the simple task of changing a tire. Keep in mind that while the task may seem to be simple, many hazards are sometimes not as apparent as one would imagine.

For the purposes of this assignment and hazard analysis, you will assume you have had a flat or blowout while driving alone, on a busy highway or road, and in inclement weather (i.e., rain, snow, fog). You may also choose whether this incident involved another vehicle.
Ensure you fully read all of the instructions. There are two documents to be prepared and submitted:

For Part A, you must complete the following:

1. Step 1: List the specific manufacturer instructions for a vehicle of your choosing that you currently own (e.g., car, motorcycle, truck, ATV, RV, boat trailer) for the changing of a tire. The list does not have to be verbatim, but the critical sequence of events should be included.

2. Step 2: Refer to your list of specific instructions and as you review the section in Chapter 9 of your eTextbook titled A Hazard Analysis and Risk Assessment Guide as well as Appendix 2 of OSHA's Job Hazard Analysis to determine each hazard and associated risk. Keep in mind you are looking to identify which conditions could be causal factors when changing a tire. You must identify at least five hazards.

3. Step 3: Use the Risk Assessment Matrix, including Probability and Severity Codes, Table 9.9 in Chapter 9 of the eTextbook to complete the risk assessment of each hazard in your list.

4. Step 4: Evaluate the hazard, associated risk, and risk assessment code (RAC) for each step/task in your list as you complete the Unit V Job Hazard Analysis Form. Ensure you have identified at least five hazards.

5. All sections of the form for each step/task must have an assigned RAC. As an example, if you determine that the occurrence probability for a step is Occasional and the severity of consequences is Critical, the RAC would then be S - Serious.

For Part B: explain the reasoning behind how you completed the Job Hazard Analysis Form. Address at least the following:

1. Briefly summarize the conditions you have chosen for this scenario (i.e., location, type of inclement weather, single or multi-vehicle incident).

2. Discuss the concepts of risk and hazards and the types of risk as they apply to this scenario.

3. Explain how the components of risk management were used to identify the hazards and determine the risks.

4. Conclude Part II with a summary of how the applied process and resulting insight achieved in this activity can be universally applied in workplace safety management systems (SMS). What value does this process add to an SMS?
