Initial management by the er team


Assignment task: Patient and family goals for Andrew. Over the past 30 years, he has been brought to the emergency department 3 times for episodes of diabetic ketoacidosis, where he is treated and discharged. He continues to drink alcohol (3-6 beers a day) and smoke (1 pack a day). Today, he woke up with an inability to move his right upper and lower extremity with mild slurring of speech and trouble swallowing. He was able to call an ambulance and arrived at the ER. His HgA1c (hemoglobin A1c) is 11.3% and blood pressure is 187/110. The ER team acquired a history, performed a physical exam, ordered imaging, and made the diagnosis of a stroke. He has a large amount of medical problems (listed above), and he has a medication list from his partner that is shown below, although he is unable to remember the names of which medicines he is actively taking, since many pill bottles at home are half-full. The next day, despite appropriate initial management by the ER team, Andrew's symptoms of weakness and slurring of words persists.
