Improving diabetes care for undocumented immigrants


Assignment task:

The health promotion activity was focused on improving diabetes care for undocumented immigrants. Nurses have a role in educating and advocating for our patients to ensure they receive the best quality of care. Immigrants lack health insurance and access to health care, preventing them from receiving services for chronic conditions such as diabetes which can lead to long-term effects (Leining et al., 2023). The program included volunteer internal medicine physicians, diabetes nurse educators, case managers, pharmacists, and other medical staff. The focus of the study was to provide immigrant patients with diabetes the resources needed to decrease their HbA1C in one year (Leining et al., 2023). This included the supplies required to check their blood glucose and the medications needed to treat their diabetes. The program connected patients with other resources in the community such as the food banks (Leining et al., 2023). Each patient was seen every 3 months unless they were high risk due to medication regimens. If so, those patients were seen weekly. Follow-up appointments were made at the current appointment to ensure that patients would attend the next appointment. Patients were provided diabetes self-management education and support by the diabetes educator (Leining et al., 2023). The program was successful because of the resources and the follow-up provided to the patients. The resources allowed patients to receive the medication needed to treat their diabetes and the follow-up kept patients accountable. Something that could have been improved upon is the medications that were available to the patients. Also, giving the patients resources like a primary care doctor for them to follow up with on other chronic conditions that they may have.

The MSN-prepared nurse has a huge role in providing patients with the proper education based on their literacy regarding diabetes and diabetes treatment. As nurses, it is important to be aware of the patient's history and social background when providing education as it is patient-based. Not all patients have access to the same resources. The use of health literacy techniques like asking patients what they think caused the health problem, using the teach-back method, and knowing what family is involved in their healthcare planning is important (Murdaugh et al., 2019, p. 119). The use of these techniques will provide a general understanding of their strengths and level of health literacy. Getting to know our patients beyond their health conditions is important in order to provide patient-centered care. Need Assignment Help?
